The best TRT tracker apps in 2026

Eight apps built for tracking testosterone replacement therapy, compared on what they actually do: level curves, bloodwork, privacy, platforms and price. We make OptiPin, so read this with that in mind. We link every competitor's listing so you can check each claim. Where the others win, we say so.

The short version

OptiPin – the pick on iPhone if you want your testosterone, estradiol and DHT estimates recalibrated to your own bloodwork, with the data kept on your phone and no account.

Regimen – the pick if you need Android with cloud sync, or want the biggest review base of any dedicated TRT tracker.

TRT Plus – the cheapest over time: a one-off $29.99 lifetime purchase, on iOS and Android, with a bloodwork-anchored level model.

TRT Tracker: Injections log – the simplest log if you want doses, sites, symptoms and labs recorded and little else.

What none of these apps can do. Every level curve below is a model estimate drawn from your doses. It is not a blood test and it cannot tell you whether a dose is right for you. Protocol changes belong with your prescriber. If you get chest pain, shortness of breath, or a swollen, painful calf, get urgent medical care rather than checking an app. A rising hematocrit or blood pressure reading, or a marked change in mood, is something to raise with your clinician, not something to adjust around on your own.

OptiPin – calibrated hormone estimates, kept on your phone

OptiPin started as a testosterone tracker in May 2025 and still goes furthest on the hormone side. Its testosterone curve uses ester-specific models (cypionate, enanthate, propionate, undecanoate, Sustanon and gels among them), then recalibrates against your own lab results, so each blood draw pulls the estimate toward your physiology. It also estimates estradiol and DHT from your protocol and any aromatase inhibitor you log. None of the other apps here claims an estradiol estimate. Around that sits the day-to-day tracking: 115 mapped injection sites, vial inventory with run-out dates and low-stock alerts, bloodwork you can scan from a lab report or pull from Apple Health, a Medication Impact view that lines dose changes up against your check-ins and Apple Health data. Its AI insight summaries then write up, in plain language, how your doses, labs, symptoms and Health data are moving together. There is also a PDF summary for your doctor. There is no account. Data lives on the phone, with optional iCloud Sync across your own Apple devices since August 2026.

Where it falls short: iPhone only, so no Android and no web app. It is also the densest app on this list. If you run one testosterone ester and nothing else, most of it is machinery you will never open.

Best for: iPhone users who want estimates anchored to their own labs, especially anyone also running an AI, hCG, a peptide or a GLP-1 alongside testosterone. Get OptiPin on the App Store →

Since September 2026 it also reads your Apple Watch nights (or Whoop, Oura or Garmin data in Apple Health) into a nightly recovery score scored against your own last 60 nights. Health Trends then compares recovery on mornings after injection days with other mornings. It also compares HRV and sleeping heart rate in weeks on each compound against weeks off. Regimen's Signals also sets dose changes against Apple Health sleep and heart rate, but gives no recovery score.

Regimen – the cross-platform pick

Regimen runs on iPhone, iPad, Mac and Android with cloud sync between them. It pulls weight, sleep and heart data from Apple Health or Google Health Connect. That reach is its biggest genuine advantage. On Android it is the strongest option we found. Its Signals feature correlates dose changes with 50+ daily check-in markers and wearable data. It also plots level curves for a large compound library. At 459 US ratings it also has the largest review base of any dedicated TRT tracker.

Where it falls short: it needs an account and your history lives on its servers. Its level curves are built from published half-life data. We found no claim that they are re-fit to your own labs, or that it models estradiol or DHT. It is also the priciest of the big four at $7.99 a month or $49.99 a year.

Best for: Android users and mixed-device households. Regimen publishes its own TRT roundup, which ranks Regimen first and OptiPin third. Read both. Our head-to-head is at OptiPin vs Regimen.

TRT Plus – lifetime price, deep modeling

TRT Plus is the oldest app here (on the App Store since 2023) and the only one sold as a one-off $29.99 lifetime purchase, which makes it the cheapest by a distance if you plan to track for more than a few months. It runs on iOS and Android. Its listing claims a multi-route PK model covering injectables, orals, peptides and creams, peak-to-trough modeling, dose titration plans, photo scanning of lab reports with an AI bloodwork report and level predictions anchored to your actual bloodwork. That last claim puts it in the same bracket as OptiPin on calibration.

Where it falls short: a small, mixed review base (4.0 from 19 US ratings). Its listing also leans heavily on "enhanced" protocols and its own branded models, which we could not check independently.

Best for: anyone who hates subscriptions and Android users who want more modeling than Regimen offers.

TRT Tracker: Injections log – the plain log

From the same developer as the Anabolic Steroid & TRT Tracker, this one is scoped to TRT and nothing else. It logs injections, gels, patches and pellets, rotates sites on a body map, tracks symptoms and side effects and charts blood work over time, including hematocrit. It is straightforward and does what it says.

Where it falls short: no pharmacokinetic level curve that we could find and no peptide or GLP-1 support. It was last updated in March 2026.

Best for: someone on a single prescribed protocol who wants a clean record for their clinic visits.

Four newer apps worth knowing

Three launched in 2026 and MyTRT in 2025. All have 11 or fewer US ratings, so treat them as promising rather than proven.

Himcules (10 ratings, 5.0) runs on iOS and Android with per-ester PK curves for cypionate, enanthate and propionate, three reminders per shot, vial supply warnings, 14 mapped sites, iCloud backup and a one-tap doctor PDF. The listing says a premium tier is still coming.

Calibrum (11 ratings, 4.9; $39.99/yr) puts an AI hormone coach and doctor-visit prep summaries on top of dose, symptom and lab tracking, with no account required.

MyTRT (1 rating; $9.99/mo or $99.99/yr) is on iOS and Android, scans lab reports into 55+ biomarkers with AI and draws a PK curve with adjustable half-life. Regimen's roundup ranks it second.

Trough (2 ratings; $9.99/mo or $49.99/yr) is private by design with no account. It adds streaks, levels and badges to the daily check-in. PK curves and bloodwork are in the paid tier.

In the UK, Ireland, Australia or Canada?

Labs in the UK, Ireland, Australia and Canada report testosterone in nmol/L, not the ng/dL used in the US. A total testosterone of 15 nmol/L is about 432 ng/dL. OptiPin has a setting that shows bloodwork in SI units (nmol/L, mmol/L), so your results read the way your lab printed them. None of the other listings or websites we checked on 29 September 2026 mentions which units it uses, so enter one real result before you commit to an app. Our testosterone unit converter handles one-off conversions.

How to choose

Start with your phone. On Android, it is Regimen, TRT Plus, Himcules or MyTRT, because the rest are iOS only. On iPhone, decide what you want the app to tell you. If you want an estimate that moves toward your real labs, including estradiol and DHT, that is OptiPin. If you want a one-off price, TRT Plus. If you only want a tidy record, TRT Tracker or Trough. OptiPin and Regimen are both free for one compound with no card, so the cheapest honest advice is to log a week in two of them and keep the one you open without thinking.
